Integrated Care provides evaluation, treatment, rehabilitation, and work-status coordination for work-related musculoskeletal injuries. Our goal is to help employees recover safely while keeping employers informed about functional abilities, restrictions, and return-to-work planning.

Please provide:
• Employer authorization for evaluation or treatment
• Date and description of the injury
• Workers’ compensation carrier and claim information, when available
• Employee job title and essential physical functions
• Employer or claims contact information
• Any existing work restrictions or medical records
What employees should bring
• Government-issued photo ID
• Employer authorization and injury paperwork
• Claim number and adjuster information, if available
• Current medication list
• Relevant medical records, imaging reports, or restrictions
Important scope and safety information
Integrated Care’s injury-care program focuses on musculoskeletal conditions. We do not provide emergency care, sutures, or removal of foreign objects from the eye. Serious bleeding, suspected fractures or dislocations, significant head injuries, chest pain, breathing difficulty, severe chemical exposure, eye emergencies, or other urgent conditions should be directed to an emergency department or appropriate urgent medical facility. Call 911 for a life-threatening emergency.
Work-status communication
Following evaluation, the authorized employer or claims representative may receive appropriate work-status information, restrictions, treatment recommendations, and follow-up plans. Medical information is handled according to applicable confidentiality requirements.
Scheduling and billing
