Spirometry is a common breathing test that measures how well a person moves air into and out of the lungs. It may be used for occupational health surveillance, respirator-related evaluations, baseline testing, or follow-up when a worker reports respiratory symptoms.
What does spirometry measure?
During the test, a spirometer records the amount of air exhaled and how quickly it leaves the lungs. Two commonly reviewed measurements are:
- Forced Vital Capacity (FVC): the total amount of air forcefully exhaled after a full breath.
- Forced Expiratory Volume in one second (FEV1): the amount exhaled during the first second.
A qualified healthcare professional interprets the measurements in context. Spirometry alone does not diagnose every respiratory condition, but it can help identify abnormal patterns or changes that may require further evaluation.
Why is spirometry used in occupational health?
Workers may encounter dusts, fumes, gases, vapors, or other airborne hazards that can affect lung health. Occupational spirometry can establish a baseline and help track lung function over time. Testing is required under certain federal standards for workers with specific exposures, including some work involving respirable crystalline silica, cotton dust, and coal mine dust.
Spirometry may also be considered for physically demanding work, respirator use, or symptoms such as persistent cough, wheezing, chest tightness, or shortness of breath. The exact testing schedule should follow the applicable standard and the recommendations of the occupational health professional.
How to prepare for the test
Follow the instructions provided with your appointment. Unless your clinician gives different directions, these steps may help:
- Wear comfortable clothing that does not restrict deep breathing.
- Avoid a large meal immediately before testing.
- Tell the technician about recent illness, surgery, chest pain, dizziness, or respiratory symptoms.
- Bring a current medication list and follow instructions about inhalers or other respiratory medicines.
- Avoid smoking or vaping before the test as directed.
Do not stop prescribed medication unless the ordering clinician specifically instructs you to do so.
What happens during spirometry?

- The technician explains and demonstrates the maneuver.
- You sit or stand in the testing position and may wear a nose clip.
- You take the deepest breath possible and seal your lips around a clean mouthpiece.
- You blow out as hard, fast, and completely as possible.
- The maneuver is repeated, usually at least three times, to obtain acceptable and repeatable results.
The test requires strong effort and careful coaching. You may feel briefly lightheaded or winded. Tell the technician immediately if you experience pain, severe shortness of breath, or significant dizziness.
After the test
A healthcare professional should explain the results and determine whether follow-up is appropriate. In occupational surveillance, individual medical information remains confidential; the employer typically receives only the work-related information permitted or required for the program.
