Maintaining your CDL medical card is essential for your career, but the DOT physical can be stressful. At Integrated Care in Omaha, we see hundreds of drivers every month. Most pass with flying colors, but others face unexpected delays.
To help you stay on the road, here are the top 5 reasons drivers fail or get "deferred"—and what you can do to prepare.
1. High Blood Pressure (Hypertension)
This is the most common reason for a shortened medical card (e.g., 3 months instead of 2 years).
- The Fix: Take your prescribed medication on the morning of your exam. Avoid caffeine, energy drinks, and nicotine at least two hours before your appointment.
2. Missing Sleep Apnea Documentation
If you use a CPAP machine, the examiner must verify that you are using it consistently.
- The Fix: Bring a compliance report from your machine covering the last 90 days. Most modern machines allow you to download this via an app or a USB drive.
3. Unmanaged Diabetes
Drivers with diabetes can pass, but their blood sugar must be under control.
- The Fix: Bring your most recent A1C lab results (performed within the last 6 months). If your A1C is above 10%, you may face delays.
4. Vision Issues
It sounds simple, but many drivers forget their glasses or haven't had their prescription updated.
- The Fix: If you wear contacts or glasses for driving, you must bring them. We recommend an eye exam if you’ve noticed any blurring lately.
5. Missing "Clearance Letters" for Heart Conditions
If you’ve had a heart attack, stent, or major surgery, the DOT examiner needs to know you are fit for the physical demands of driving.
- The Fix: Bring a letter from your cardiologist stating you are cleared to drive a commercial vehicle, along with your most recent stress test results.
